All Dayton Eye Surgery Center physicians are Medicare Participating providers.  Our state-of-the-art surgical center is also a Medicare approved facility.
 
Many medical insurance companies, as well as HMO's, require pre-authorization for medical services. 
 
Please consult with your insurance company or primary care physician to obtain a pre-authorization for services, if required, prior to the date of your appointment at Dayton Eye Surgery Center. 
 
 
Our business office will contact you prior to surgery concerning financial arrangements and to obtain all the information needed to verify and process your insurance benefits.  We will notify you in advance of expected patient responsibility including any unmet deductible and co-insurance. 
 
To provide our patients with convenient payment options, we accept cash, cashier's checks, patient personal checks, MasterCard, Visa, American Express, Discover, and Care Credit.

Ambulatory Surgery Center Pre-Payment Policy
 
Thank you for choosing the Dayton Eye Surgery Center for your surgical care. 
 
Depending on your insurance benefits, you may have a co-payment, co-insurance, deductible or other out-of-pocket fee when having a procedure in an outpatient facility.  The Dayton Eye Surgery Center policy states that all patients having outpatient procedures in our facility are expected to pay these out-of-pocket expenses prior to or on the date of service. 
 
In some cases, you may be contacted by our Patient Advisors to go over our pre-payment policy in detail.  If you have multiple insurance policies, or schedule within 24 hours and were not contacted, you will be billed for the out-of-pocket responsibility after your insurance claim is paid.  Any amounts discussed are estimates and do not guarantee payment in full. 
 
We encourage all patients to contact their insurance company to confirm what their financial responsibility is.  Any questions about your covered benefits should be directed to your insurance company. 
 
After your claim has been processed with your insurance carrier, you will likely receive multiple bills.  These may include:
 
Professional Charges:  Professional charges are the fees that are paid to the physician for performing the surgery.
 
Facility Charges:  Facility charges are fees from the building where the surgery is performed and covers the cost of medications, implants, and supplies.  These come from the Dayton Eye Surgery Center. 
 
Anesthesia Charges:  Anesthesia charges are the fees from the anesthesiologists' services during your surgery and recovery.  For questions before your surgery, you can call Anesthesia Services Group Billing at 1-800-951-7850. 
 
Here at the Dayton Eye Surgery Center, we offer our patients the following financing option: 
 
Care Credit:  The application can be made online using their website www.carecredit.com or contact them directly at 1-800-365-8295. 
 
Copays/Deductibles must be paid in full prior to or by the day of surgery/procedure.  Copay/deductibles more than $500 may utilize the 3-month payment program with half down and the remaining balance paid across 3 months with a credit/debit card on file.
